As a courtesy we will verify insurance benefits and inform you of your insurance coverage.  However, it is important that you understand your benefits prior to receiving services and may want to call your insurance company to clarify benefits. We are able to check benefits and eligibility by obtaining your insurance company name, identification number, date of birth, and the insurance provider’s phone number.

If we are not an in-network provider for your insurance plan, your insurance company will offer benefits for an out-of- network provider at a different percentage rate. 

We also accept Medicare, auto insurance claims, worker’s compensation, and liens.

For patients that do not have and insurance, we offer cash rates.
